Our Care Case Form Widget allows your congregation to initiate a Care Case from your Portal website or main church website.  Notifications in MinistryPlatform can be setup to be triggered based on new Care Case submissions, notifying Pastoral Care staff of each new case.

Our Care Case Form Widget can be configured to allow one or many Care Types. You can also preset the Care Type allowing you to embed the Widget on a page specifically designed for Prayer Requests and then embed the Widget on a different page designated for Hospital Visits or Grief Care.
